Pilot program partnership improves health outcomes of sponsored elders with diabetes

A partnership between Unbound and Chronic Care International on a three-year noncommunicable diseases pilot program has improved the health outcomes of participating sponsored elders living with diabetes in the Philippines. The pilot could pave the way for the establishment of a low-cost model of care for diabetes management that can be replicated in developing countries to assist individuals living in poverty.
